I am aware that University of St. La Salle has collected and stored my personal data during my admission. These data include my demographic profile, contact details like addresses, email address, and landline/mobile numbers.

I express my consent for University of St. La Salle to collect, use, record, disclose, transfer, store, organize, update, monitor, and/or process my personal information.

I agree to personally update these data through an email request as needed. Towards the efficient management of the school records, I authorize University of St. La Salle to manage my data for data sharing with accredited company/industry partners and government agencies.

To ensure the protection of my rights as a data subject, I understand that University of St. La Salle shall warrant to me the following rights:

• Receive notices regarding changes in the above-cited purposes for processing of my personal data
